連続して5日以上ご欠席される場合、前月10日までにこの長期欠席フォームをご送信ください。
次期の明細にて、欠席期間中のランチ・スナック代をお返しいたします。

Please send this form by the 10th of the previous month, if your child will be absent for 5 or more consecutive days.
We will refund Lunch/Snack fees in the next Tuition.
